Miniature medical history card

The File of Life® is a program that aims to make vital health information available to emergency responders in the event of a medical emergency. The "File" is a miniature medical history card that is filled out and kept in a red plastic sleeve with a magnetic strip. The magnetic sleeve is placed on the outside of the refrigerator in a visible place.

The File of Life® details a person's name, emergency medical contacts and insurance policy information. It details current health problems, current medications and dosages. It lists allergies, recent surgeries, your doctor's name and phone number, your health care proxy information and your religious affiliation.

Who is the File of Life® Program for?

The File of Life® is for anyone in the community who would like to ensure that, in the event of a medical emergency, emergency responders have immediate access to potentially life-saving personal health and medical history information. People who are at an increased risk for a medical emergency, such as seniors or community members with special needs, are especially encouraged to participate in the program.

How do I participate with the File of Life®?

Simply obtain a File of Life magnetic sleeve and history card; fill out the card and place the card in the sleeve on your refrigerator. Emergency responders in Manchester are trained to check for this.

File of Life supplies are available from all Town fire stations or by calling Manchester Fire-Rescue-EMS at (860) 647-3266.
